What is the biggest derby in world football?

Boca v River seems to be the consensus, and I'd be inclined to agree.

Al Ahly v Zamalek in Egypt gets a nod. By all accounts, it is pretty intense with several deaths over the years.
 
Went to Madrid last year for the last derby at Atletico's ground. Walked into the ticket office on the day of the match and bought 2 tickets....
 
I do miss our games against poopsmuff. Can get a bit tasty at times.
Maybe not the biggest in the world and not a same city derby but, there is real hate between the two!
