There's a scene where a fella's head/face is crushed by repeated blows with a fire extinguisher, there's also a 15 minute scene. It's a mental film, unwatchable in places.

The scene from "Pan's Labyrinth" where he pounds the other guys nose in with the wine bottle is pretty rough too.
