What differentiates top level players and the others?

What I mean, is to trained football eyes (e.g. scouts, coaches) how do they decide if someone is good enough for a particular level or not?

For example, I understand that physical atrributes make a difference, and pace and height are preferred attributes esp. in the modern game.

Why are some players who have performed well in the Championship not able to make the step up to be playing consistently in the PL? Is it the inability to make the right decisions quick enough? Is it lack of hard work, or lacking the right agent?
 
Consistency, self belief and natural talent. Combine all that with hard work and you get the finished article.

I presume that the process of "rating a player" is similar to the statsheet you get with games like football manager. Meaning that a trainer would watch a player and rate him at various skills as to get the complete picture and determine where he's best fitted also according to the team strategy. So basicly you would look how fast he runs and put a score on that, how long he can run, how strong he is, how high he can jump, how good he is at tackling or passing, etc.

Rating a player to level then usually depends on the "total score", most top players have "high stats across the board". Although speciality must also be taken into account, you can have a player that is overall less skilled but is one of the best at a few particular things and then depending on the team and how they play that specialist can be just perfect and also provide more value to them even if in another system he wouldn't fit the level at all. The same might apply if a players stats are simply great for his role. Picture the kind of CB that has no ball playing abbilety's at all, not much of a footballer then but if he's really strong and tall and effective at defense he will be rated anyhow and then it just requires that you have say a defensive playmaker nearby that can take the ball of him and bring it further because if you gonna rely on that defender to also play the ball then youre gonna get dissapointed with him.

Most top players have a broad amount of skills that are far higher than the average on their level and typicly concentrated to their role. You take CR for example well he can shoot and score, he can dribble, he's fast and physical, he can head a ball in, but the most notable about his all roundness as attacker here is that he's also one of the best at all those things. A fantastic dribbler, a great finisher on foot and with the head, great physical attributes and a few things more.
