Just speculation, but could either be that they hear it from VAR (broadcasters have access to VAR voices), they could see the 4th official "type" in the number on the substitution board, or they enquired the 4th official about added time.

Assume the protocol is that the ref tells the 4th official how much time will be added on by the 89th minute for them to set the board up.

Touchline reporter then asks 4th official how long there will be and relays to the match commentator.
