Worst casting in a film

I watched The Client the other day and the blurb made a point that the young boy in the film was a complete unknown and the director did not want a child actor. He wanted someone real. The boy cast was called Brad Renfro. Unfortunately he died of drugs in his twenties. I'm making a big assumption but I'd guess he may not have gotten into drugs had he not been cast so for him it might have been some bad casting.
 
Ray Winstone whenever he has to play someone with a different accent to a cockney.

His American accent in the Departed was hilariously bad.
