I think Azpilecueta made no attempt to jump for the ball, wasn’t looking at the ball, was looking at the man and decided not to jump to stop Nane from reaching the header.
Mane jump up and sideways to body check Azpilecueta in mid air, but because Azpilecueta didn’t jump there was contact with the head.
Mane then immediately pulled his arm away when he felt the contact.

Not how I see it but a decent explanation either way, how many players do you see jump using one arm for leverage?
