It can happen, but often it is a case of penalty or dive. Refs rarely book a player in those circumstances.

It doesn't matter whether two yellow card incident's occur within a few seconds of each other or not, if they are both deserving of a yellow then 2 yellows should be shown and the player should be off (just like if you commit a last man foul in the first few minutes of a cup final it is still a red card but that's another ref and another story).
Only diving to win a penalty.
I don't believe Bale "Dived" in this incident anyway. More like he went over avoiding the challenge.
It's diving full stop, not just for penalties.
But I do detest when people say: "If it's not a penalty book him then".
