Working in the aircraft industry I have witnessed it on many occasions, gaffer taping and tie wrapping someone to a seat mid atlantic, while being spat at and having death threats is truly terrifying for other passengers.
I was once on a flight with an incredibly drunk Royal Marine who's CSM had to vouch for him (a very big deal). They want to be on first so there's a space for their bag right above them, if you're last on your bag could end up halfway down the plane.there is something about airports that turns sane, rational people into raving lunatics, one example of this is the incessant need to all crowd around the boarding gate like the plane is gonna leave without you if you're not first on.

I think incidents like this are still quite isolated, most of us don't experience them thankfully, but that is because there are so many flights every day, and the majority pass off uneventfully.
