Crystal Palace. They apparently play in Selhurst, which is in the borough of Croydon. And which isn't referenced in their official name, only their ground. You could say the rags as well. Though I suppose you could say that Newton Heath is in Manchester.
I must admit United was why I added in (or originally played) and I don't think Everton are in Everton any more. There must be others - some may argue that Bolton are no longer in Bolton - and QPR don't play in Queens Park,

QPR is less than 3 miles from Queens Park. The club was formed after Christchurch Rangers merged with St Jude’s, in the Queens Park and Kensal areas of London. Most of their players originally came from Queens Park, hence the name.
Arsenal, on the other hand, having bribed their way into division One in the back of a blackmail threat to expose match fixing by the rags and the dippers, had already moved over 12 miles from south of the river Woolwich, to north of the river Highbury.
They then, of course, routed Huddersfield of their manager and most of their players and were known as the ‘bank of England club’ as they broke transfer record, after transfer record and record wages paid.
None of it was, of course, ‘earned income’…

and (being pedantic) three miles away is not actually in Queens Park.
