Completely depends on where you got it done and how they would price it up such as whether they are charging per name or per letter. I'd say per name would cover atleast 10 letters, so you're probably gonna get charged for 2/3 names.
Numbers are short numbers so will probably be about £3 for each pair.
Would expect to get it for less than £30 but depends on whose behind the till, and if they are in a charitable mood may do it for less.
