I’ve printed a copy off as I couldn’t find a way of completing it online,and as far as I’m aware,it’s just nhs covid pass for travel and the sworn statement for entry into France and the ground

Apparently worth getting the french equivalent of the NHS app for access to restaurants\bars etc
