For me this has to be Anton Chigurh in No Country for Old Men. Bardem is excellent and the unpredictability of a Cohen film just increases the suspense.

Mads Mikkelsen was equally as good in the Hannibal TV series.Anthony Hopkins as Hannibal Lecter in The Silence of the Lambs. Absolutely chilling and unnerving performance.
I agree, that's about as disturbing as it gets in a film.Great shout re Bardem. The scene with the old guy in the hardware shop. Extremely unnerving.
