The 3rd Kit description from the official premier league handbook is:
"Colours: Shirts: Ghost green with
black trim / Shorts: Ghost green
with black trim / Socks: Ghost green
with black trim"

There's a graphic as well but its in PDF format and not much better than in that tweet above.
