Why is there no outburst about this, surely its worse than wearing a t-shirt with a gun on it. Not a good example.

There is a falicy that smoking makes you unfit.
It's not true.
If you train a lot then it takes many years for smoking to reduce your lung capacity and it only takes a short time off the smokes to regain full capacity.
There is very clear evidence that smoking increases the risk of cancer. But this does not mean it makes you unfit at a young age.
