Very few hull/life loss accidents are attributed to single mechanical failures anymore. The steep lessons learnt from many previous disasters in history have been credited to magnificent improvements in aircraft design safety and maintenance improvements. The majority of major aircraft accidents are operational external influences like weather or pilot/engineer error. No other transport industry has so much regulation as the airline industry. The rigor and dedication by all within the aviation sector are rewarded when approx 6 airplanes land safely every second of the day, somewhere in the world.
