If the Police had been made aware of the threat of something happening and not acted on it, they will need to have their part in this tragedy examined and the findings made public.
It sounds like the Police will not be coming out of this favourably.
They always refer themselves to the IPCC in a case like this. It doesn't mean they have done anything wrong, in most cases it's just a paper exercise, dotting the I's, crossing the T's, just to make sure they have done everything properly.
