Theres an 80s horror film that scared the shit out of me, my cousin was well into his "video nasty" stuff. All i can remember is at the end the person who is meant to be a young girl is stood there screaming, covered in blood and its revealed "she" has a cock and has been raised as a girl. It will be nothing now im sure but that image stuck with me when i was about 7

Sleepaway Camp.

I'm assuming the girl having a cock is the "twist", no pun intended, probably not worth watching now.

im an absolute horror nut,i own about 800 an got a youtube channel devoted to em.some great shouts on here.
The Changeling with George c scott.
Martyrs.
Suspiria.

but theres a 1990 tv movie called The Haunted that is the only film since i was a kid to genuinly freak me out.had some angry ghost shoutin on a tape recorder.check it out.

Top of the list has to be Suspiria, unfortunately the dubbed soundtrack doesn't stand up too well after all these years, but the suspense, and music do.

Beat me to it mate. Wasn't it withdrawn pretty quickly from UK cinema's because of subliminal imaging/soundtrack ?
