Not really what the op asked but the most famous substituted substitute has to be that bloke who claimed to be George Weah's cousin and got a game for Southampton. Pretty sure he only lasted about 10 minutes before being hauled off again. Not sure what happened to the player who replaced him but that could be a possibility.

I think the alleged cousin was called something like Ali Diah.
