Can anyone help?

Want to make a banner template, but useless at photoshop or banner making.

Got it all worked out, just need some bright spark on here to knock up a template so I can email it to a printer.

Any help would be greatly appreciated. No rush, but I'd like the banner ready for Charity Shield.
