If Trident was used, the most likely scenario is it would come down the submarine commander at the time.
There are specific instructions locked away in each sub on what the course of action is to be - as the chances are, London wouldn't actually exist anymore, and there's no telling what state comms would be in. Quite how the commander is determine that London is no more - I don't know.
There are some underwater communications plus the more obvious radio / sat, and maybe some other technologies, but logically, in order for the sub to know that London is gone, it must either a) be specifically told so, or b) assume so due to a lack of communication. How long the commander would wait after communications failed, I don't know, and how he'd know which nation attacked us, I don't know (unless he's getting downlink information from some tracking satellite.
It's supposed to be silence on Radio 4 for a set number of days.
There are also supposed to be instructions on each sub, written by the Prome Minister. When we get a new PM each sub recieves new instructions. So if you're a smart Johnny Foreigner looking to bomb the UK you assassoinate the PM. The singel sub that is at sea then needs to get new instructions some how. You pin point the hand over from Postman Pat and launch.
