So 25 medals for the 25 man squad kinda thing. Cool was worried ederson would miss his shot at the golden gloves, still hope foden and Diaz get minutes, plus a start for yaya on the last home match would be nice.

Have i missed something here.
That's his League Cup medal he got, despite being on loan at Everton - he was obviously gone by the time of the final but played in the earlier rounds so hence why he gets a medal!
Everyone who has made 5 appearances MUST receive a medal, the club can decide who receive the rest, the PL Board have to approve more than 40 medals and will only do this if 39 players have made 5 appearances!!

The League Champions shall further receive 40 commemorative medals to be presented by the Club to its Manager and to such of its Players and Officials as it thinks fit provided that any Player who has entered the field of play in a minimum of five of its League Matches that Season shall receive from the Club a commemorative medal. Additional medals may only be presented with the consent of the Board which shall only be given if the total number of Players who have entered the field of play that Season in a minimum of five of the Club’s League Matches exceeds 39.

It's not great, but it's not that bad. Club gets 40 medals to give out as they like, but they are required to give one to every player with 5+ appearances (so they can't screw someone they don't like if they played 5+ games). If more than 39 players played 5+ games, the League will give out more medals so that everyone with 5+ appearances definitely gets one.This is horrendously worded.
