That's his League Cup medal he got, despite being on loan at Everton - he was obviously gone by the time of the final but played in the earlier rounds so hence why he gets a medal!

It's not great, but it's not that bad. Club gets 40 medals to give out as they like, but they are required to give one to every player with 5+ appearances (so they can't screw someone they don't like if they played 5+ games). If more than 39 players played 5+ games, the League will give out more medals so that everyone with 5+ appearances definitely gets one.
