1. Proposal: i.e it must pass a 2/3 voting of Congress ( both house and Senate): I.e. the representatives of the people.
2. Ratification: Then the legislatures of 3/4th of the States have to ratify it. Again, doubly representing " We the People"...
In so far as there is a loophole in the 22nd amendment, it isn't one that I think can or will apply to Trump... But that loophole will probably be shutdown by the courts if anyone ever used it.

As for the 'coup' in 2014, then president Viktor Yanukovych turned dictator and became Putin's puppet. He jailed former presidential candidate Yulia Tymoshenko, curtailed press freedom and presided over widespread corruption in government. In November 2013 he refused to sign an association agreement with the EU under pressure from Putin despite Ukraine's parliament approving it by a huge margin. Protests against him intensified and in February 2014 nearly 100 protestors were killed when government forces opened fire on them. After signing an agreement with the opposition on 21st February Yanukovych immediately fled Kyiv and the following day Ukraine's parliament voted 328-0 to remove him from office because he had abdicated and withdrawn from his constitutional duties. Even some of his own party voted for his removal. They also voted to release Tymoshenko from prison and his own party formally disowned him. On the 24th he fled to Russia via Crimea with the help of Russian forces as confirmed by both Yanukovych and Putin. He now lives in comfortable exile in a house owned by the Russian Ministry of Internal Affairs near Moscow all paid for by the Kremlin. Yanukovych was a traitor.

Example:
Barack Obama runs as the V.P. Candidate to some chumb, lets say your Governor Pritzker. Pritzker gets in office after winning and then resigns for some reason....
Obama then would be able to ascend to the presidency and invariably have a 3rd term.
That scenario is the only potential loophole to the 22nd amendment as written. Nothing else...
