I think refs in those situations (where the keeper is pressured into losing the ball), if they are sure there's a foul, will want to whistle quickly, so there's no goal to disallow. In this case he was possibly a little unlucky in that the goalkeepers weird handling meant the decision turned out to be really clear cut.

It's both, not the machinery per se but the design of the system. And all this has had a negative effect on the referee's decision-making.
