OP - Sorry to hear what has happened to you. Sounds like you have suffered some nasty injuries there, especially if they have had to plate the collar bone. Hope you recover quickly and get back to work, life, cycling etc.
To cut to the chase, I'm a 20 year qualified PI solicitor. I'm not touting as I'm currently not practising, but I'm happy to advise you if you want to pm me.
Having read most of the posts above, I would say:-
Don't try and instruct Eversheds. They are a national commercial law firm.
Personally I wouldn't instruct Shoosmiths.
Avoid claims management companies like the plague.
Check your own insurance policies for legal expenses insurance, as it could save you the success fee deduction that goes along with no win no fee.
Try and find a good, reasonably local solicitor who specialises in RTA claims, and who is ideally is a member of the Association of Personal Injury Lawyers. I used to know someone who worked at Thorneycrofts in Macclesfield (although he's long gone from there and he was a red), but I think they are a decent outfit not too far from where you are.
With your level of injury don't try and deal with the driver's insurer yourself - it's not a bit of bent metal and whiplash.
Although it sounds like the driver is a complete cnut who deliberately injured you, don't expect the police to prosecute him for anything other than careless driving, if they even do that. However, that should not affect any civil claim you have, and as the police attended the scene you should not have any issues over obtaining the driver's details, including insurance details.
You don't need to do anything immediately about claiming. Concentrate on your recovery etc. However, sometimes initiating a claim early, especially if liability is not going to be disputed, can unlock rehab and/or early interim payments of damages for physio if you are struggling to get it on the NHS.
Good luck and as i say just pm me if you want any further advice and to chat it over.
