I really can't see any family having a picnic on barren rocky ground in 40C heat yards away from his body.

I've had it once when working in Africa, and by all accounts I was all over the place, even though I though I was fine enough to carry on working. Mine was combined with dehydration, due to grafting in the midday sun in over 45 degree heat (due to the reflected heat from the concrete) and high humidity due to the proximity to the edge of a jungle. We physically couldn't drink enough water and our body salts were seriously depleted.

Raised core body temperature is the main issue for sure. A rise in core temperature really creeps up on you, and then when it tips the threshold you usually have a rapid onset of symptoms which are hugely disorientating. By raising your core temperature too high you effectively disable your body's thermostat, at which point you get the biological equivalent of thermal runaway.
