I think it's because we think a job is done and lose concentration. That is partly to do with a lack of self belief and the winning mentality to go the extra mile.

Our passing and ball retention has always let us down. Therefore, we are not able to mount a sustained period of pressure on an opposing team. Therefore not getting into a winning situation, unless the other team is complete shite.
