Your not on your own I’m going through something similar myself I’ve opted not to have chemotherapy and opted for self ?? forgot what it’s called but blood tests and scans instead, the specialist was in agreement, fingers crossed for us all going through the similar thing.

My BIL had the radiotherapy and hormones and it didn’t suit him, I think your wise to do what you think is best for you especially when you’ve got other health issues going on, hope you managed to enjoy Xmas day we are nearly into New year and spring will soon be here again :) after this endless wet 2024 which might be affecting the team it’s never stopped raining.

4.6 isn't too high but it's sensible to get it checked out.

For reference mine was 23 when I was diagnosed last year, but all the doctors that I spoke to said that PSA wasn't the most useful indicator of Prostate Cancer (I'd argue that it was in my case!).

Maybe they can rule out cancer with a scan but the only way to definitively diagnose that something is cancer is with a biopsy.

Anyway, in your case there are lots of things that it could be, so I wouldn't get too worked up over it at the moment. Just take it one step at a time.
Thanks mate. Yep, I'm not overly worried as things stand. Like I posted the other day, there are plenty of things it could be. I'm presuming you had a biopsy yourself? If so, how was it?

So far so good my blood test came back below normal and a MRI scan of the pelvis/abdominal came back (no sign of invasive cancer spread) so currently dealing with colonoscopy when I understood I didn’t have any colon left.? :)
I find it stressful because it feels like one appointment after another and I’m waiting for an appointment at Salford royal for an Ileostomy reversal if it’s possible since last October :(
I’ve been very lucky so can’t complain but it’s all so confusing

How are you doing because your a bit like me taking your chances trying to stay as well as you can ?

Fine for me as I was put under a general anaesthetic - first time I've ever been in hospital, so that was the biggest worry for me. I had it done under my company's private medical cover but I don't know whether it's the same on the NHS. Afterward sit was a little sore for a few days but no pain so there's no worries there.

Some good news which is good mate, I’m glad to hear it.
Medical appointments are a way of life as we get older.

I find sitting in my lounge day after day is ok until sits not. I meet up with two lots of friends one lot for breakfast once a fortnight and one lot for dinner once a fortnight, this helps, keeping connected with the outside world helps.

I feel ok, I’m not in any pain from my prostrate but I use to have the reflexes of a thousand cheetahs. But no more I’m afraid, I’m losing my balance and becoming more unsteady. I’m going to try and start chair exercise see it that helps.

Chair exercise might help or if you can manage a little walk round the garden or the road every day just to keep the blood flowing. You have to stay safe though so a word with the doctor about your balance, as sometimes balance can be an inner ear issue. Have a word with the surgery see what they say.

I will mate. I struggle to walk because I have necrosis of the hips. lol just another load of bollocks to put up with.
 
Not surprised, they can do them all at once so the cost of a dedicated psa check is saved.
It is still a dedicated psa test they take 2 sleeves of blood one being purely for a psa you miss my point the surprise which is welcomed was how proactive they were in offering it which is countering a lot of people’s experiences

Not sure about whether Tamsulosin can affect PSA readings.
I have been taking it for BPH over the last 4 years and having my PSA level checked 6 monthly and none of the urologists has ever mentioned this possibility.

Other things can affect PSA levels. Ejaculation and bladder infections have been mentioned to me previously. My recorded levels have usually been between 14 and 17 these last few years. One reading of 37 was put down to a bladder infection.

I don’t think anyone knows for sure yet what causes an enlarged prostate.
Age is definitely a factor.

I’m sure that someone on here may be able to give you some more information about your query.

My PSA readings don't match those on here.
I started last Christmas at 2.8. Then 3.1. Then 2.9.
I was told my 3.1 was the low end of a high reading etc. So what's the high end?
My latest has come back 4.9.

Unfortunately PSA levels fluctuate for all of us it seems and they are not a reliable indicator of prostate disease.
As I mentioned in my previous post, prostates will enlarge with age. There are charts that show mean readings according to your age which you can find on Google.

What’s a “normal” PSA level?

Doctors tend to categorize PSA levels in terms of “elevated” or “non-elevated,” rather than “normal” or “abnormal.” That’s because the range of what’s normal varies by age. Technically, someone with any PSA level can have prostate cancer; that’s not the only thing we look at when assessing a patient.

Generally speaking, though, PSA levels for men who are:

  • Age 60 or older: should be at or below 4.0 mg/mL
  • Age 59 or younger: should be at or below 2.5 mg/mL
The average PSA for men in the younger group is <1.0 mg/mL

What is considered a dangerously high PSA level?
Technically, there is no upper limit. PSA levels can go into the tens or even hundreds of thousands. But generally speaking, these are the ranges of elevation:

  • 4-10: minimally elevated
  • 11-20: moderately elevated
  • 21+: high elevation
Your doctor will likely investigate anything over 10. Anything greater than 20 starts pointing more toward cancer. Anything above 50 is usually a very strong indicator of prostate cancer. Levels that high are usually not due to an infection or some other thing going o

No. But a PSA above 4.0 in anyone over age 60, or above 2.5 in anyone younger than 60, will likely prompt additional testing

There have been a few studies on reduction of psa levels when taking tamsolsin, and it would appear the jurys out on this one, royal marden conducted some, saying that it can marginally affect results. A drug that can affect psa is finasteride with the ability to lower psa readings to under a half in the first 12 months of taking. To adjust for psa monitoring on finestiride, the consultants have to double the score.

Of course two different drugs entirely, with tamsulosin being an alpha blocker, relaxing muscles in neck of baldder/prostate, whereas finesteride is an alfa reducter inhibitor that shrinks the prostrate. Both have their own main side effects, with tam users experiencing retrograde ejaculation and a minority of fin users experiencing eractile disfunction

BPH .. enlarged prostrate, is more likely age related. The web says it can be caused by hormone changes.

As Two Gun Bob says above two drugs are often used, there are no indications that Tamsulosin affects PSA readings but Finasteride is known to lower PSA readings and doctors will normally make an allowance for that when evaluating the PSA results.
 
I took Tamsulosin for a few weeks after my brachytherapy, just to ensure that the urine flow was not impeded by the swelling.

Couldn't wait to get off it - every time I stood up I felt dizzy. The side effects went away a couple of days after stopping it.
