Marconi (I think) developed a supposedly sophisticated homing torpedo a good number of years ago. They test-fired it at a static target the first time but it couldn't pick up the target and just went around aimlessly till it ran out of fuel.If it had been live....no more submarine or Royal Navy personnel left on it.
Leaked info or not, there's zero use for a system that takes out the firing site.
So they made adjustments to the software and tried again. This time it was more successful in picking up a target. However the target it locked onto was the sub that had fired it. Fortunately it wasn't armed.

Heard a retired Admiral today on the news say in all its time we have only test fired 12 Trident missiles............. the last two were mis-fires

Correct. You can get a leak of nuclear msterial, but not a detonation....and even then if a full on "nuke" warhead, the chance of it detonating is virtually zero.

Yeah I think there have been 190 odd in total - makes you wonder why we have such a large failure rate
