If you read the article, it says it has happened in the past where it wasn't a loaded gun.Someone must be accountable for bringing a loaded gun on to a film set. Supposed to be firing blanks apparently. How on earth does an accident like this happen.

This is what caused Bruce Lee's lad to be shot whilst filming
"It was later determined that the gun used had earlier fired a round that caused a cartridge to become lodged in the barrel. When blank rounds were later fired, the cartridge was dislodged and released."
