That's the sleeve badge, not part of the club crest.

Bayern have like six stars in which one is awarded per five titles, but that isn't awarded by the DFB, that's just added themselves. It's only Poland and Italy I think do it so it's like official.

UEFA have never awarded stars as part of a badge, only a sleeve one known as the Badge of Honour. City will however get a title holders badge next season in CL games.
 
Forest have 2 stars on above there badge
That's put on there by the club, it's not officially given. Ipswich have like three.... One for their league title, FA Cup win and UEFA Cup.

Panathinikos have a self awarded silver one just for reaching the final. A target of ridicule from Olympiakos supporters.
 
The Raggies self-awarded themselves one in 1998 with that kit they only wore for CL games.

With national teams it's not FIFA recognised and it's not required, but they do it anyway for every World Cup they win.
 
Plenty of teams have, or have had, stars on their shirts. Forest of course, Huddersfield had three around the time we did, Bury had two and I also seem to remember Ipswich used to have three stars above their badge last time they were in the Prem. There are probably more, and that's just English sides.

Yep, Ipswich's are for their league title, their FA Cup and UEFA Cup.
