Passenger locator form for arriving in Portugal for the official trip

What did you put down as the airline? Jet2?

Not filled mine in yet but on email from sportsworld it says the airline is air tanker

The email sending the Certificates was Certificates@dam-health.com might be worth emailing them too whilst ringing.

Excellent. It's all a bit of a joke. The certificate I have now is definitely what the airline want to see, glad I chased it up, just hope It doesn't cost others their trip

Just had my test done at DAM health in Sheffield - anyone else had the test done at DAM where it was just the throat and not the up the nose job? Slightly concerned now...;

If your flight has an 'LS' prefix, that's Jet2
