Did Frank Worthington also fail one at Liverpool around the same time as Asa ?

Plenty,
Ruud Van Nistelrooy failed one the first time United wanted to sign him
Demba Ba failed one at Stoke last year
Marveux failed a Liverpool medical before joining Newcastle
Asa Hartford at Leeds, Cissokho at AC Milan, Hinchcliff at Spurs, Finnan at Hull, Milito at Real, Hartson at Rangers. etc.

There was a famous one in January when George Boyd failed a medical due to not having 20/20 vision.
