Berwick rangers play in England and have won the Scottish league div 2.
Soviet Union teams don't feature in my answer, which is restricted top tier titles so lower divisional success by Berwick in Scotland and the Welsh clubs in England doesn't count either.

I think we need a bit of a clue so is it a case of a team winning in a foreign country by completing a league season after a country has broken up; for example when Yugoslavia or Czechoslovakia split into several smaller nations? Or is it a club that regularly plays in a foreign league as Cardiff, Swansea and Newport do?

In one case not after a country has broken up but rather the reverse. The other club falls firmly into your second category.

Just checked and Derry City won the League Of Ireland 88-89 and 96-97 as I suggested in an earlier post. That must count surely?

Yes, Derry City is the second club. They've won the FAI Premier Division twice, in 1988-89 and 1996-97 - sorry I missed your mention earlier!
And Rapid Vienna is the third, after the Anschluss, Rapid Vienna won the German league in 1941 - which lancs blue also posted in my Bermuda Triangle portion of the thread. Well done both of you, I'll try to keep up better.
