Foul throw ins

Players constantly standing off the field while releasing the ball after their hands have crossed the line aren't throwing the ball legally.
In what sense?

You used to need part of both feet behind the line, then it changed to on the line. After the throw-in the ball is immediately in play, and the ball is often over the pitch when released, there is nothing against that.

So long as it's thrown and not dropped it's fine.

It is a bit odd that "The ball is in play when it enters the field of play" when the ball can be in the field of play before it's been thrown.

Where the feet go hasn't changed. It's been the same (part of each foot on or outside the line) since 1938, possibly earlier.
