Put him in the hospital until he is stabilised on meds then put him in prison , problem solved

Until the paranoia resurfaces and he starts making weapons and concealing them or assaulting other inmates for no reason.
 
He was off his meds , when people like him are on their meds daily they are more stable, medical fact, go bother someone else

Christ. How do would you ensure he takes his medication and doesn't misbehave It's also far more normal to be paranoid in prison due to fact he will be surrounded by other people with short tempers and violent mindsets.

It felt like prison occasionally but, when I was in hospital, at least one nurse stood over me whilst I took about half a dozen different tablets. They were in a lockable section of my cabinet for which they held the key.

I’m assuming medication is supervised by prison staff.

That's true but there are restrictions on when medication can be forcibly administered. Even in secure hospitals.

If the man is so unwell to be forcibly medicated for life, why is prison a more appropriate place than a secure hospital?

In a secure hospital, they are likely to have better facilities, more staff with the appropriate level of training and experience and a more therapeutic environment.

It's not likely this guy, who has a history of violent random outbursts and avoiding medication will want to comply and prison undoubtedly will make him worse even when medicated. Anti-psychotic medication mitigates the symptoms of schizophrenia it doesn't eliminate them entirely.
