Milner played twice for England. Rested I would think and the players on the bench are better than him.

matter of opinion really, I would say hes good enough to be in the side based on games like the derby

Just remember Balotelli kept his cool in the FA Cup final against Stoke despite being elbowed in the face by Huth and some Stoke tackling. He even delivered a great performance.
