Unanimous means all 3 have to agree. A 2-1 majority does not apply.

What is the procedure, who actually asks the panel to review incidents?

FA wait for report, if referee says they saw but did not see extent or did not see, they instruct panel.

Panel of 3 ex-referees.

All 3 agree it was violent conduct -> charge
any other event -> no charge
