I've read that if you quit using the dashboard (Xbox) or the PS button (PS3) whilst in an online game then it resets all of your stats including the rank, so maybe that is the problem?

Nah, I've seen on the activision forums that there was a server hicup at between 1 and 4 so those three hours have been lost. If you didn't log in or out between those hours you wouldn't notice.
