I think one of the major contributing factors was that duckinfield had little or no football crowd experience and definitely not at the level required to be a match commander of a match of that magnitude. I’ve read that he didn’t even have a proper hand over from his predecessor mr Mole who was removed from post a few weeks previously after a stunt involving some officers went wrong.The police had years of practice packing people into those pens. The crushes they caused were the reason we only went into the seats at Hillsborough and I still find it astounding that a large number of people involved that day didn't serve jail sentences.
When it stared to go wrong he didn’t react correctly and a chain of events soon spiralled out of his control and he panicked and made decisions that lead to people being crushed to death.
However there were a number of warning signs in the years leading up to that semi and should have been heeded but unfortunately were not.
