Manchester derby - Where would you rank it?

It depends in what context.
I would rate it pretty highly but that is because I personally hate United, Celtic or Rangers fans would obviously have a different view.

I've heard that the Red Star Belgrade derby with Partizan (I think) is pretty brutal

Genoa vs. Sampdoria gets pretty insane. But yeah, I think our particular rivalry is much more interesting because of the footballing content and on field intensity then because of the spectacle the fans make of it. Which I think is a good thing. You don't need to start fights and set off fireworks in stadiums to make a rivalry a rivalry.
