Ive spoken to pathologists about this after people have been tragically found and retrieved. Falling in suddenly means they are not going in feet first, or head first if diving expecting to come up straight. They lose all sense of where they are and what way up they are. In panic they are breathing in rapidly, swallow water, cough and choke, swallow water in which is made worse by taking more in as they gasp. They thrash, tire, and clothing gets sodden making weight they cant counter as muscles freeze and cold water shock happens. Ive seen it too many times, in shallow water even, but mainly in rivers. Freezing cold water in winter after a sudden fall is lethal
