One team gains 3 points. The other team loses 3 points.Just means that it's a Very Important Match for both sides. Usually used when the two are direct rivals for the title.
But also used when both are fighting relegation, and one will leapfrog the other if they win. But it is only three points. I've checked. Our upcoming match with Arsenal at the Etihad will only gain either side a maximum of three points. Things can still happen after that…
