The TV angle is further away than the linesman was and it was obvious it had gone out.

No flag came because the directive is essentially "if you're not sure, don't give anything".

Was only a matter of time until a serious injury occurred because of it.

That isn’t the directive in that situation, though. No flag came because the assistant fucked it up
