He has always had a stutter he has struggled to overcome. Its the sort of thing that people use to call him sleepy Joe. Really high level political discourse to mock a stutterer rather than the content of what he says.

This article is from September. The cognitive decline stuff was a myth spread by the Trump campaign. But because Trump is Trump, he had to overblow it and it didn't work.His surrogates used the above term suggestively, Trump wasn't even supposed to use the word Dementia.

Simple explanation. His stammer got caught by a trigger word "secretary" and then he diverted to the folksy stuff. He has done this before, when he is stammering over one word, he doesn't force it and compound the issue. Hence he didn't say Pppp... Pentagon.
